New Families Registration Information

For new registrations, please bring the following to the office:

• Registration Form

• Student Birth Certificate

• Baptismal Certificate

• Reconciliation Certificate

• Confirmation Certificate

• Copy of Parent ID

• Vision screening

• Current Physical records

• Current Dental records

• Registration fee of $125 (non-refundable)

• Emergency medical form

• Extended Program application (if applicable)
